- 如需修改或補列印申請書，請至修改及列印申請書頁面。
If there is a need to modify or reprint the certificate application form, please visit the Modify and Print Certificate Application Form page.
- 確認線上申請資料無誤後，請把相關申請資料做為公文附件，發送公文至主管機關窗口進行初審作業審核。
Once you have confirmed that the online application information is accurate, kindly include the relevant application documents as attachments to the official document. Please send the official document to the supervisory authority's officer for the initial review and processing.
